They are connected to the nasal cavity through small openings that allow mucus and secretions to be expelled from the cavities into the nose, and also allow ventilation of the cavities. If these cavities become blocked, this can cause the mucous secretions and germs to accumulate inside the cavities, leading to the development of inflammation. Lack of ventilation also leads to increased pressure inside them. Sinusitis can be acute or chronic. It is usually accompanied by sudden blockage of the sinuses, leading to congestion of mucous secretions inside the sinus cavity, which turn into pus, and the pressure inside these sinuses increases, causing severe headaches and other symptoms. Most episodes of sinusitis occur through infection.

What are the Different Types of Sinusitis?

There are two main types of sinusitis: acute and chronic. Acute sinusitis is inflammation that lasts for less than four weeks, while chronic sinusitis lasts for more than 12 weeks.

How Do You Get Acute Sinusitis?

In the majority of people, acute sinusitis occurs following infection with illnesses such as a cold or the flu. The viruses that may spread to the sinuses cause colds and the flu. In a small number of cases, bacteria become additional factors indicating the presence of an infection that begins as a result of the virus entering. In some cases, the infection spreads to the maxillary sinus due to an infected tooth. In some people, one or more factors may contribute to the occurrence of sinusitis, making them more susceptible to infection. These causes include:

1- Nasal allergy
2- Other causes resulting from blockage in the drainage channels of the nose, such as nasal polyps, objects entering the nose, or facial injuries.
3- Asthma.
4- Cystic fibrosis.
5- Disorders resulting from inflammation, such as Wegener’s Granulomatosis, which is considered one of the small vessel vasculitis diseases. Wegener’s disease affects all organs of the body, most importantly the lungs and kidneys.
6- Pregnancy, which makes you more susceptible to nasal inflammation.
7- Smoking.

What are the Symptoms of Acute Sinusitis?

The symptoms of acute sinusitis include the following:

– Pain and tenderness in the affected sinuses. The pain and throbbing usually worsens when bending the head forward.
– A general feeling of nasal blockage. Loss of the sense of smell for a period of time. On the other hand, some people experience a runny nose. If the discharge is green or yellow in color, you are likely to have a bacterial infection in the sinuses.
– Fever and a general feeling of being unwell.

Other symptoms that may occur include:

– Headache
– Bad breath
– Pain in the face between and around the eyes, the forehead, or the back of the head, depending on which sinus is affected
– Pain in the teeth and upper jaw
– Sometimes swelling and redness of the skin covering the affected sinus
– Coughing and a feeling of fullness or pressure in the ears, and fatigue

When is Medical Attention Required?

If you are experiencing the following symptoms:

1- High fever (> 102.5º Fahrenheit or 39.2º C)
2- Sudden and severe pain in the face or head
3- Double vision or difficulty seeing
4- Confusion and difficulty thinking clearly
5- Swelling or redness of one or both eyes
6- Stiff neck and difficulty breathing

What is the Treatment for Acute Sinusitis?

The initial treatment for sinusitis aims to relieve symptoms, as almost everyone will improve within the first seven to ten days. It is recommended to take over-the-counter pain-relief medications such as Acetaminophen or Ibuprofen to relieve pain.

Rinsing the nose and sinuses with a saline solution several times a day can help reduce pain caused by congestion.

Nasal decongestant sprays can be used, including Oxymetazoline, which is a nasal decongestant, and Phenylephrine, to treat temporary congestion. However, these sprays should not be used for more than two or three days due to the risk of rebound congestion.

If symptoms do not improve within seven to ten days, arrangements should be made for a medical evaluation, and further treatment may be needed.

Nasal Glucocorticoid sprays can help reduce swelling inside the nose, usually within two or three days. These medications have few side effects and significantly relieve symptoms in most people.

Are There Any Complications from Acute Sinusitis?

Sometimes acute sinusitis can develop into chronic sinusitis, which is the most common complication. Chronic sinusitis causes symptoms similar to acute sinusitis but they last for a longer period.

There are other complications that are rare, but can nevertheless be serious. For example, the infection may spread from the sinuses to the eye, bones, blood, or brain. The occurrence of severe complications is estimated at approximately 1 in every 10,000 cases of acute sinusitis. They are more common with inflammation in the frontal sinus, and children are more susceptible to complications compared to adults.

What are the Treatments Used for Chronic Sinusitis?

If you have an underlying problem that was the cause of chronic sinusitis, treating it will usually help relieve and manage these symptoms. If you have chronic sinusitis and you are a smoker, you may find that stopping smoking improves symptoms. This may especially be the case if you also suffer from allergies. Divers who have nasal or sinus problems should be aware of the potentially serious consequences of sinus barotrauma (Sinus Barotraumas) resulting from the difference or change in pressure between the sinus cavities and the surrounding environment. Flying on an airplane may increase pain if there is a blockage in the sinus drainage channels, due to the change in air pressure in the aircraft, whereby the pressure between the sinuses and the external environment cannot equalize as a result of the blockage.

Long-term treatment may be recommended using nasal sprays or nasal drops that can help reduce inflammation. Steroid sprays or drops in particular may be of great benefit and assistance in cases where it is believed that allergies play a prominent role in the condition. A longer course of treatment than usual compared to antibiotics may be recommended (for example, for a period of two weeks). It is usually the ENT (ear, nose and throat) specialist who is best placed to determine whether a prolonged course of antibiotic treatment would be beneficial for you or not. The use of antifungal medications is necessary if you have a fungal infection of the sinuses.

Surgical intervention is used mainly when no improvement occurs with the medical treatments mentioned above. The main purpose of surgery is to improve the drainage of the affected sinuses.

This most commonly performed procedure is called functional endoscopic sinus surgery (FESS) for the treatment of sinus disease and nasal polyps. This requires the surgeon to insert an endoscope into the nose. The endoscope used for this procedure is a thin, rigid instrument containing lenses. The endoscope allows a detailed, magnified view of the inside of the nose so that the surgeon can see the opening of the drainage channels in the sinuses. The surgeon can then remove any tissue that is obstructing the drainage of the affected sinuses. This can improve sinus drainage and ventilation, and help restore normal sinus function.

Sinusitis is an inflammation of the sinuses located inside the bones of the skull, which are air-filled cavities surrounding the nose and eyes.
